You must upload all of your NON-PASSWORD PROTECTED documents to the DOCUMENTS tab so we are able to complete the enrollment/registration process over the phone. The system WILL NOT allow you to save and submit your information if you do not have all of your documents. Please click here for a list of required documents. You will need to download and complete the Registration Checklist, Domicile Statements and any Release of Records Consent Forms and upload them during the enrollment process. Failure to do so will result in a cancelled appointment and will delay your enrollment and registration.
